Mechanism of Action

Sodium bicarbonate is an alkaline agent that directly neutralises hydrochloric acid in the stomach through the reaction: NaHCO₃ + HCl → NaCl + H₂O + CO₂. This rapid acid neutralisation raises intragastric pH within minutes — faster than any other antacid or acid-suppressive agent — providing immediate symptomatic relief of heartburn, acid dyspepsia, and oesophageal burning. The CO₂ generated provides a secondary eructation effect that physically expels intragastric gas. Sodium bicarbonate is also a urinary alkaliniser — after absorption, it increases urinary pH, reducing uric acid and cystine crystallisation in the renal tubules and providing an unfavourable environment for certain uropathogens.

Indications

Acute heartburn and acid dyspepsia (fastest-acting antacid), acute urinary tract discomfort (urinary alkalinisation reduces dysuria), prevention of uric acid crystalluria and renal stones in hyperuricaemia, alkalinisation for drug toxicity management, and as a component of oral rehydration regimens. Also used in ENT practice as a sodium bicarbonate gargle for pharyngitis and tonsillitis.
