OMEKEY 20

Omeprazole is the original proton pump inhibitor — the first PPI to reach clinical use and the global standard against which all subsequent PPIs have been compared. It irreversibly inhibits H⁺/K⁺-ATPase on the gastric parietal cell secretory membrane through acid-catalysed activation in the canaliculus, converting to a sulphenamide that forms covalent disulphide bonds with the pump enzyme.

Omeprazole 20mg achieves approximately 85–90% reduction in 24-hour gastric acid output — sufficient for peptic ulcer healing and GORD symptom control in most patients. Its effects are dose-dependent: 20mg once daily for GORD maintenance and peptic ulcer healing, 20mg twice daily for H. pylori eradication (or the higher 40mg dose), and 40mg twice daily for Zollinger-Ellison syndrome.

Omeprazole undergoes significant CYP2C19 metabolism — approximately 15–20% of South Asian patients are poor CYP2C19 metabolisers (achieving higher omeprazole exposure and better acid suppression), while 5–10% are ultra-rapid metabolisers (achieving inadequate acid suppression at standard doses). This inter-individual variability has clinical implications but is manageable through dose adjustment.

Peptic ulcer disease (gastric and duodenal), GORD and reflux oesophagitis, H. pylori eradication (as PPI component of triple therapy), NSAID-associated ulcer prophylaxis and treatment, Zollinger-Ellison syndrome (higher doses), stress ulcer prophylaxis, and laryngopharyngeal reflux. As the most established PPI globally, omeprazole has the broadest evidence base across all acid-related conditions.
