Mechanism of Action

LAXOHIT combines two complementary laxative mechanisms — a synthetic osmotic disaccharide and a bulk-forming soluble fibre — providing comprehensive constipation relief that addresses both stool consistency and colonic transit simultaneously.

Lactitol Monohydrate (10g per sachet) is a synthetic disaccharide (galactose + sorbitol) that is poorly absorbed from the small intestine, reaching the colon essentially intact. In the colon, it is fermented by colonic bacteria to produce short-chain fatty acids (acetate, propionate, butyrate) and gas (CO₂, H₂, methane). This fermentation lowers colonic pH and increases osmotic pressure in the colonic lumen, drawing water into the colon and softening stool. Unlike lactulose (the more commonly used osmotic laxative), lactitol produces less gas and flatulence because its fermentation kinetics are more gradual and produce fewer gas byproducts — making it better tolerated with less bloating. Lactitol also acts as a prebiotic, selectively stimulating Lactobacillus and Bifidobacterium growth while suppressing Clostridium and other putrefactive bacteria — improving overall microbiome health alongside laxative action.

Ispaghula Husk (Psyllium, 3.5g per sachet) is a natural soluble dietary fibre derived from Plantago ovata seeds. It absorbs water to form a viscous gel that: increases faecal bulk (stimulating peristalsis through mechanoreceptor activation), retains water within the stool (preventing dehydration and hardening during colonic transit), and lubricates the intestinal wall (reducing friction during defecation). Ispaghula's gel-forming action also slows gastric emptying and glucose absorption — providing secondary metabolic benefits beyond laxation.

Indications

LAXOHIT is indicated for constipation across multiple aetiologies — the dual-mechanism combination providing more comprehensive relief than either agent alone.

Chronic Constipation (Primary): Functional constipation in adults and elderly — lactitol's osmotic softening combined with ispaghula's bulk formation addresses both the stool consistency and transit time components of chronic constipation.

Hepatic Encephalopathy: Lactitol is a validated alternative to lactulose for HE management — acidifying the colonic environment (reducing ammonium production by urease-producing bacteria), increasing nitrogen incorporation into bacterial biomass, and reducing ammonia absorption. Multiple clinical trials confirm lactitol equivalence to lactulose for HE, with better tolerability.

Irritable Bowel Syndrome (IBS-C): Ispaghula husk is the only dietary fibre with Level I evidence for IBS symptom reduction — particularly effective for IBS-C where stool normalisation is the primary therapeutic target.
