Esomeprazole 40mg (the S-isomer of omeprazole, with higher bioavailability ~89% versus omeprazole's ~35%, providing more consistent and potent acid suppression) + Domperidone 30mg SR (peripheral D2 antagonist prokinetic, 12-hour sustained release). Esomeprazole's isomeric purity eliminates the R-omeprazole inactive isomer, delivering the full pharmacological dose of active PPI compound — achieving superior intragastric pH control compared to racemic omeprazole 40mg.

Erosive oesophagitis, severe GORD, functional dyspepsia with significant acid component, H. pylori eradication (PPI component at 40mg for maximum pH elevation and antibiotic efficacy), peptic ulcer healing, and gastroparesis with GORD. The 40mg esomeprazole dose is reserved for moderate-to-severe acid conditions requiring stronger suppression than 20mg formulations.
