Mechanism of Action

SIMSOFT is a dual-component antacid and anti-flatulence suspension combining a buffered antacid with a defoaming agent to simultaneously neutralise gastric acid and eliminate gas-related gastrointestinal symptoms.

Magaldrate (400mg per 5mL) is a hydroxymagnesium aluminate compound — a unique, chemically specific antacid that acts as a pH-buffered acid neutraliser rather than a simple alkali. Unlike simple antacids (magnesium hydroxide, aluminium hydroxide), magaldrate undergoes a stepwise ionisation in the stomach: it neutralises HCl progressively, releasing magnesium and aluminium ions only in proportion to the amount of acid present. This buffered neutralisation mechanism maintains gastric pH between 3.5 and 5 — sufficient to inactivate pepsin (which requires pH below 3.5 for activity) and relieve acid symptoms, without raising pH above 5 where the achlorhydric rebound effect (hypersecretion post-alkalinisation) occurs. The result is more sustained, physiologically appropriate acid control compared to conventional antacids.

Magaldrate also directly adsorbs pepsin, bile acids, and lysolecithins — the non-acid components of peptic injury — providing protection beyond simple acid neutralisation.

Simethicone (60mg per 5mL) is a synthetic silicone polymer (polydimethylsiloxane) that acts as a surface-active defoaming agent. Gas bubbles in the gastrointestinal tract are stabilised by surface-active agents in intestinal contents — simethicone reduces the surface tension of these gas bubble films, causing them to coalesce into larger bubbles that are more easily expelled by normal intestinal peristalsis. The result is faster, more complete gas elimination and relief of flatulence, bloating, and abdominal distension. Simethicone is pharmacologically inert — it is not absorbed, metabolised, or excreted systemically, acting entirely within the GI lumen.

Indications

SIMSOFT is indicated for gastrointestinal conditions characterised by acid-related symptoms and/or excess gas formation where both antacid and anti-flatulence effects are clinically beneficial.

Dyspepsia and Non-Ulcer Dyspepsia: Epigastric pain, heartburn, belching, and bloating — the most common gastrointestinal presenting complaints. SIMSOFT provides rapid symptom relief for both acid (magaldrate) and gas (simethicone) components of functional dyspepsia.

Peptic Ulcer Disease: Adjunct antacid therapy for symptomatic relief of peptic ulceration during the healing phase initiated by PPIs or sucralfate. Magaldrate's pepsin adsorption provides additional mucosal protection beyond acid neutralisation.

GORD (Gastro-Oesophageal Reflux Disease): On-demand antacid therapy for breakthrough reflux symptoms in GORD patients on maintenance PPI therapy.

Gastritis: Acute and chronic gastritis — magaldrate's buffered acid neutralisation and pepsin adsorption relieve the burning epigastric pain of gastric mucosal inflammation.

Flatulence and Bloating: IBS-associated flatulence, post-operative gas pain, dietary indiscretion-related bloating — simethicone provides rapid mechanical gas relief.

Paediatric Use: Post-feed gas pain and colic in infants — simethicone drops are widely used for infant colic, and the suspension format allows paediatric dosing.

Post-Endoscopy: Gas insufflation during endoscopic procedures causes significant post-procedural abdominal distension — simethicone reduces this effectively.
